Required forms may be found on our website for download. Required forms and information: Group Registration, Participant Registration, Brothers' Keeper Health Form, insurance card and date of last tetanus shot.

roup Registration: send with initial payment (one for each group)

Participant Registration: one for every youth and adult. This must be turned in by June 1 to guarantee t-shirt size. After that date, all shirts will be XL. 

Brothers' Keeper Health Form: one for EVERY youth and adult. No camper will be allowed to stay without a notarized medical form.

Insurance Card: one for EVERY youth and adult.

Date of Last Tetanus Shot: EVERY medical form must have this information included.

Medical Forms and Insurance Cards must be received at Parchment Valley PRIOR to the beginning of camp to assure a smooth registration process. Do not wait until camp to avoid errors or missing information! If we do not have the correct information in time, you may not be able to remain at Brothers' Keeper.
